Observation

2a) debris is seen in the baby play yard from tress that could cause a choking hazard. 2c) Power cords were seen accessible to children as well as long looped cords and a dangling strap at the changing table, as well as a string attached to a toy in the baby yard. 2d) plastic bags were seen throughout the buildings. 3a) cracked plastic at the bubble in the fence created a sharp edge, staples from a pinch guard were exposed due to the guard not being secured on one side, low tree branches, the posts have splintered surfaces. 3e) rubber pad placement created a gap, posing a tripping hazard outside. several "nails" securing the turf were raised posing a tripping hazard 3f) a heavy garden type tool (stanley pick/hoe) was seen placed on a shelf outside, above the children's shoe storage. 3g) fence panels and siding had chipping paint 4c) the rooms are kept dark during nap time with blackout curtains pulled closed not allowing adequate natural light into the program for supervision purposes. 5f) An extension cord is in permanent use in a bathroom for the light.
